If your child requires an over-the-counter medication for fewer than 10 days, this form must be filled out following the package instructions and signed by a parent or guardian. If the medication will be needed for more than 10 days, please complete the prescription medication form instead, which requires authorization from a healthcare provider. The school nurse reserves the right to request a healthcare provider’s authorization for any over-the-counter medication when deemed necessary.
